If you want to schedule a first visit with us, call and ask to speak to our intake coordinator. She will review your needs and help guide you through the appointment process.
 
 
2. Prepare for your first visit.
 
To help make the most of our time together, you need to arrive at your first appointment with some specific paperwork.

If you are seeking services from our Education Consultant, scroll down to the Education Consultation Forms and follow those directions. Otherwise following the directions below.

Please download, print, and complete the forms requested by us. Typically, you’ll need to complete several registration forms and an evaluation form and bring all of them with you to your first appointment. The "Blue Ridge Policies" is yours to keep. If you have any questions about how to fill out your forms, please call us. Bring your completed forms and evaluation(s) to your first visit.
